Is something writing my notes for me?
No. The objective section is arithmetic on the trials you tallied. Dictation is speech to text under a signed BAA. S, A, and P are yours. Nothing drafts a SOAP note.

Where does the dictation audio go?
To a transcription service under a signed BAA, and it is deleted there before the transcript returns to you. Nothing is stored and nothing is used to train anything.

- Drafting your note for you. S, A, and P are your words; O is arithmetic on your tally.
- Rewriting or cleaning up dictation. It lands verbatim, and you edit it like any text.
- Editing after signing. Signed means fixed; corrections go in a follow-up note.
- The full widget set on the phone. Counters, talk-time, summary entry, and the cued-versus-independent split line are web portal features today.
Questions SLPs Ask
- What does the objective line actually look like?
- One line per goal in the form “16/20 (80%) at Word level, max support: Direct verbal cue”, with a second line separating cued and independent trials when a session has both.
- What if I did not tally live?
- Type the count at the end (8 of 10, with the cue level) and it is written up like any other goal, with a small “summary-entered” mark so the record is honest about how it was collected.
- Can I use my own note format?
- The four sections are SOAP. Within them the wording is entirely yours, and the objective line follows the form SLPs already write by hand.
- What happens to a note I never signed?
- It waits in the documentation queue, oldest first, until you sign it. Nothing is sent or shared from an unsigned note.
